At LHI, we are proud to help our Armed Forces satisfy the medical readiness requirements that allow them to meet any challenge. Supported by a nationwide network of providers, we provide the highest level of care with customized solutions that can support a single Service member or a group of 2,000.Through exams and screenings, we assess the overall health of Service members while working seamlessly with Government systems.Our goal is to make sure every Service member has optimum health before they deploy so they are equally ready for civilian life upon their return. We are there for our heroes, anytime we’re called upon to provide Care for Mission Critical.At LHI, we provide health care services to the U.S. Military either in network clinics, on-location at group events or through the LHI Contact Center. Our service offerings include:. Periodic health assessments.

Screening tests. Behavioral health. Physical exams. Dental exams. Dental treatment. Immunizations.

Audiology services. Vision services.

Laboratory services. X-rays and radiology services.

Periodic Health AssessmentThe Periodic Health Assessment (PHA) is a screening tool used by the armed forces to evaluate the individual medical readiness of their Service members. It can be conducted alone or can be combined with other individual medical readiness needs (e.g., dental exam and immunizations). Completed annually, the PHA consists of the following components for the Service member:. A self-reported health status.

Measurement and documentation of vitals (height, weight, BP). Vision screening. Review of current medical conditions with healthcare provider. Focused exam of identified conditions (as required). Cardiovascular Screening Program Services (as required). Recommendations for improvement of identified health conditions.

Behavioral health screen.ImmunizationsImmunizations are offered in-clinic or through group events. A:For USAR only, and only for an in-clinic appointment: you need to start the process on your AKO site. Complete the Service member portion and call Customer Service at 1-888-697-4299 to schedule the face-to-face appointment. If you are having problems accessing your AKO site, call the number above and complete the Service member portion over the phone.If you are getting your PHA at a group event, you should not complete the AKO portion of the PHA as this information will be obtained at the event.All other branches contact your command.
